Fluorescence in situ hybridization (FISH) is a method of detecting specific sequences of DNA or RNA by using oligonucleotide probes labeled with a dye that can be visualized by fluorescence microscopy .It is commonly used to identify bacterial species using a probe to detect ribosomal RNA, but it may also be able to detect antibiotic resistance genes by using probes complementary to sequences that include known mutations responsible for resistance.
The presence of point mutations in the 23S rRNA genes of clarithromycin‐resistant strains of H. pylori allows the FISH technique to distinguish resistant and sensitive strains of this bacterium in two to four hours.
FISH has also been used with blood cultures to detect K. pneumoniae strains that produce extended‐spectrum β‐lactamases and with E. coli cultures to detect the messenger RNA corresponding to the TEM genes for β‐lactamase, which confer ampicillin resistance.
A variation of FISH, peptide nucleic acid fluorescent in situ hybridization (PNA‐FISH), uses a small DNA probe (13-18 base pairs) that has an uncharged polyamide backbone replacing the phosphate backbone of the DNA .The uncharged nature and small size of the probes make them more resistant to nucleases and proteases and enhance their penetration through the bacterial cell wall.
